1. Acknowledgement and Acceptance of ToothIQ™ Privacy Policies
Symbyos is committed to protecting your privacy. This Privacy Statement sets forth the current privacy practices with regard to the information collected when you interact with this website via your computer, tablet or mobile device, or through direct interaction with Symbyos. By accessing and using this website, you acknowledge your understanding of this Privacy Statement and freely consent to the information collection and use practices described in this Privacy Statement.
2. Third Party Privacy Policies
Websites that may be linked to from this website have their own privacy statements that can be viewed by clicking on the corresponding links within each respective website. Since Symbyos has no direct control over the policies or practices of any participating merchants or other third parties, Symbyos is not responsible for the privacy practices or contents of those sites. Symbyos recommends and encourages that you always review the privacy policies of merchants and other third parties before you provide any personal information or complete any transaction with such parties.
3. Information Collected on This Website and How it is Used
Symbyos collects certain information from and about users of this website in three ways:
- From the ToothIQ™ Web server logs
- Directly from the user
- Through the use of Cookies.
a. Web Server Logs
When you visit this website, Symbyos may track information from your Web-enabled device to help administer the site and analyze its usage. The type of information collected includes:
- The kind of browser or computer in use on your device.
- Number of links you click within the site.
- City, state, postal code and country from which you accessed the site.
- Date, time and duration of your visit.
- Name of your Internet service provider.
- Web page you linked to ToothIQ from.
- Pages you viewed on the site.
Symbyos uses such information as traffic, site usage, and length-of-stay to prioritize the development and management of content, and to advise any firms with which it may have a business relationship about the type of data users access.
However, none of the information gathered through Web Server Logs is used to personally identify you.
b. Use of Cookies
Symbyos may use cookies to personalize or enhance your user experience. A cookie is a small text file that is placed on your hard disk by a Web server. Cookies cannot be used to run programs or deliver viruses to your computer.
One of the primary purposes of cookies is to provide a convenience feature to save you time. For example, if you personalize a Web page, or navigate within a site, a cookie helps the site to recognize and recall your specific information on subsequent visits. Hence, this simplifies the process of delivering relevant content. It also simplifies site navigation by providing and saving your preferences and login information as well as providing personalized functionality.
Symbyos reserves the right to share aggregated site statistics with partner companies or companies/entities with which it may have a business relationship. However, Symbyos does not allow other companies to place cookies on this website unless there is a temporary, overriding customer value (such as merging into ToothIQ a helpful site that relies on third-party cookies).
You have the ability to accept or decline cookies. Most Web browsers automatically accept cookies by default, but you can usually modify your browser setting to decline cookies. If you reject cookies by changing your browser settings then be aware that this may disable some of the functionality on this website, or it may inconvenience you on future visits to the site.
Again, none of the information contained in Cookies is used to personally identify you.
c. Personal Information From Users
Visitors to this website are required to provide certain User Information to access login protected areas of the site and to personalize some of its services to them. When you register, Symbyos will require some personal information such as name, address, email, telephone number or other relevant information.
If you are purchasing a service, Symbyos will request financial information. That information is not stored by Symbyos, because it is not necessary to create or maintain your login account. Financial information is used by Symbyos only to bill you for the services you purchased. It is not stored by, or accessible through this website. If you purchase by credit card, this information may be forwarded to your credit card provider, or a reputable, Symbyos-approved merchant services provider.
You may also be asked to disclose personal information to Symbyos so that assistance and information can be provided to you. For example, such data may be warranted in order to provide technical support and troubleshooting.
Symbyos will not disclose personally identifiable information collected from you to any third parties without your permission, except as may be required:
- to fulfill or modify your requests for ToothIQ services.
- to communicate with you about any issues with ToothIQ that you report to Symbyos.
- to communicate with you about billing or login account issues.
- to protect Symbyos from liability.
- to respond to legal process or comply with law.
- in connection with a transfer of ownership or liquidation event.
4. Use of Web Beacons or GIF Files
Symbyos may use electronic images known as Web beacons—sometimes also called single-pixel gifs—that allow Symbyos to count users who have visited certain pages and to deliver co-branded services if applicable. Symbyos may include Web beacons in promotional email messages or newsletters in order to determine whether messages have been opened and acted upon.
Web beacons may be placed by third party service providers to help determine the effectiveness of ToothIQ advertising campaigns or email communications. These Web beacons may be used by these service providers to place a cookie on your computer. This allows the service provider to recognize your computer each time you visit certain pages or emails, and to compile anonymous information in relation to those page views, which in turn enables the effectiveness of advertisements and emails to be measured, along with navigating users to this website and tracking how the site is used.
Symbyos prohibits Web beacons from being used to collect or access your personal information.
5. Accessing Web Account Information
When you subscribe to ToothIQ, Symbyos will provide you with the means to ensure that personally identifiable information in your web account file is correct and current. Currently, only your name, phone number, email address, login password and security question are accessible to you through your login account. To change any of your account information that is not modifiable through your login account, please contact Symbyos technical support at toothiq_support@symbyos.com.
6. Changes to This Privacy Statement
Symbyos has the discretion to occasionally update this Privacy Statement. Whenever this is done, Symbyos will also revise the "last modified" date at the bottom of this page. Symbyos encourages you to periodically review this Privacy Statement to stay informed about how Symbyos endeavors to protect the personal information collected through, or on behalf of ToothIQ. Your continued use of ToothIQ constitutes your agreement to this privacy statement and any updates.
7. Children's Privacy
Symbyos is committed to protecting the privacy of children. This site is not intended or designed to attract children under the age of 13. No personally identifying information is collected from any person known to be a child under the age of 13.
8. Email Communication and Related Services
Email communication with Symbyos provided through certain links on this website may be shared with employees, consultants, or other agents of Symbyos who are most qualified to address your inquiry. Every effort will be made to respond to your email in a timely fashion. All communications with Symbyos are archived for future reference.
Symbyos email is neither secure nor confidential. It is possible for other Internet users to access and view the contents of your emails while they are in transit to Symbyos. If you need to keep your communication with Symbyos confidential, do not use email.
One of the services provided to Subscribers, Authorized Users and Educational Users (defined under Terms of Use) with paid access to ToothIQ's login protected areas, is the ability to email written and video content to individuals who are patients of a Subscriber or Approved Dental School (defined under Terms of Use). Outbound emails sent by ToothIQ Subscribers, Authorized Users and Educational Users are logged by Symbyos servers, merely for tracking and reporting purposes on behalf of the ToothIQ Subscriber, Educational User or Approved Dental School. Symbyos does not publicize or share those emails with any third party, whether or not for profit; and does not use them to contact the email recipient for any purpose.
ToothIQ users may also provide their email address to access the login protected areas of the website. Further, all communication from Symbyos servers with ToothIQ Subscribers and Educational Users occurs through the Subscriber's or Educational User's email address, which are also neither shared with, nor sold to anyone outside of Symbyos.
